Player Check-In Helpful Tips

Middleville AYSO families,

As a reminder with the current rules, polices, and executive orders, every player, coach, and on field participant has to check-in every time they arrive at the fields. Below are tips that will help smooth out the process of checking in with our volunteers.

1) Fill out Daily Monitoring Form
2) Check your email for new QR code
3) Go to check-in station before fields
4) Clean hands
5) Show QR code at check-in station
6) Have participant's temperature taken
7) Have participant's hand marked
8) Have fun at soccer!

http://survey.middlevilleayso.org or http://encuesta.middlevilleayso.org/
The monitoring form must be filled out the same day as your practice/game.
These are time stamped and a form filled out on Monday cannot be used for Tuesday.

To help smooth the check-in process we can scan the QR code that was emailed to you when you completed the form.
Our scanner is unable to read the QR code if it is on a black background. Phones in 'dark mode' do not display the QR code correctly for scanning. These are just an example of what you will have in your email.
